Free Athletic Physicals for Area Students

Ozarks Healthcare is pleased to announce the availability of sports physicals at the Ozarks Healthcare Zizzer Clinic, ensuring students and community members have easy access to essential healthcare services ahead of the upcoming sports season and summer athletic camp schedule.

For West Plains High School Students and K-8 School Districts:

Ozarks Healthcare is collaborating with the West Plains School District and area K-8 schools to schedule physicals at the Ozarks Healthcare Zizzer Clinic and with area providers, providing a convenient and accessible option for young athletes.

Consent/Medical History Form

All students will be required to submit a signed consent/medical history form and provide current insurance information from a parent or legal guardian at the time of the physicals, which can be downloaded ahead of time.
