Ozarks Healthcare to Launch New Digital Forms for Patients

Ozarks Healthcare is making it easier for patients to prepare for appointments with the launch of new digital pre-registration and health forms, available starting September 15. Patients will now have the option to complete their pre-visit forms securely from their phone, tablet, or computer before arriving.

The change is part of Ozarks Healthcare’s ongoing commitment to listening to patient feedback and improving communication and convenience.

“Patients have told us they want more flexibility and less paperwork at the office,” Dr. Priscilla Frase, Ozarks Healthcare Chief Medical Information Officer, said. “This new option makes it simple for them to review and update their information when it’s most convenient, while also saving time at check-in.”

Key benefits include: saving time at check-in of appointments, reviewing and updating information at home or on the go, and reducing paperwork in the office.

Forms will be sent securely to patients ahead of their appointment via text message and/or email from a local number, with a link to complete their forms.
