Helping Patients Feel Supported at Home

For Lyda Frazier, having someone she can count on has made all the difference.

“Lance is a God-send,” Lyda shared. “Anything I need, I just call him. He’s my baby, I’ve adopted him. I didn’t have anyone coming to see me before.”

Living with chronic conditions can feel overwhelming, but through Chronic Care Management (CCM) at Ozarks Healthcare Mountain View, Lyda found steady support in Lance Landry.

As a Patient Navigator with the CCM team, Lance helps patients stay connected, informed, and confident in their care - right from home. Through the program, patients receive regular monthly check-ins and personalized support designed to help them better manage chronic conditions and avoid more serious health concerns down the road.

“By checking in with patients every month, we are able to monitor their chronic conditions more closely,” Lance explained. “This also presents an opportunity to communicate other issues that the patient may have.”

From helping coordinate care and medication refills to answering questions and communicating with providers, Lance serves as a reliable point of contact for patients navigating complex healthcare needs.

For Lyda, that support has gone far beyond routine care coordination. During the winter, when she was afraid she might freeze, Lance even helped change her propane.

“Anything I need, I just call him,” Lyda said.

In addition to ongoing communication, the CCM program provides tailored education and support to help patients better understand and manage their health at home. Because team members are dedicated specifically to CCM, patients often receive quicker responses and assistance when concerns arise.

For Lance, the impact of this work is about more than healthcare - it’s about building trust, reducing stress, and making sure patients never feel alone.
