Caring Beyond the Clinic: Chronic Care Team Spotlight

For more than 25 years, Michelle McKee, LPN, has dedicated her nursing career to helping patients feel supported, informed, and cared for. At Ozarks Healthcare, Michelle serves as part of the Chronic Care Management team at Ozarks Healthcare Family Care, where she works closely with patients who can benefit from extra guidance in managing ongoing health conditions.

In her role as a Chronic Care Nurse, Michelle helps identify Medicare patients who may need additional monitoring or support, especially those managing chronic illnesses or those who may not have strong family support systems due to busy schedules. Her day often begins by reviewing schedules and identifying patients who could benefit from the program’s extra care and attention.

Michelle also serves as an important point of contact for patients outside the clinic. She is available by phone to answer questions, address concerns that arise at home, and help patients stay on track with their care plans.

“The Chronic Care Management Program gives our geriatric group of patients extra access for help and is designed to decrease visits for chronic conditions and avoid hospitalizations by catching things early,” Michelle said.

A key part of Michelle’s work involves reviewing medications with patients to ensure they are being taken correctly. She also provides helpful tools such as pill sorters and written educational materials to support patients in managing their health at home.

Prior to her role at Ozarks Healthcare, Michelle worked in private duty nursing, an experience that strengthened her passion for building meaningful one-on-one connections with patients.

“I enjoy visiting with patients because sometimes what they need most is just a listening ear,” Michelle said.

Through compassion, experience, and dedication, Michelle plays an important role in helping patients live healthier, more supported lives.
