Supporting Patients Through Chronic Care Management

Nurse Lisa McGowan and Patient Wanda Goss

For nearly a year, Wanda Goss has had someone in her corner - someone who shows up, checks in, and helps her navigate the complexities of managing her health.

“I just love Lisa,” Wanda shared. “She just shows up when I need her. She’s always there. She has helped tremendously.”
Living with multiple chronic conditions can make even routine care feel overwhelming, but through Chronic Care Management at Ozarks Healthcare Mountain Grove, Wanda has found steady support in Lisa McGowan, LPN. From coordinating cardiology appointments to helping manage medications and ensuring necessary lab work is completed, Lisa has made a meaningful difference in Wanda’s day-to-day life.

“She’s helped with my cardiology appointments, my medications, getting the labs I need—everything,” Wanda said. “It just makes things easier knowing she’s there.”

Lisa’s role on the Chronic Care Management (CCM) team goes beyond check-ins. Each month, she connects with patients like Wanda to review symptoms, monitor progress, and catch potential concerns early, often preventing more serious health issues down the road. She also works behind the scenes, coordinating between providers, specialists, pharmacies, and insurance companies so patients don’t have to manage it all alone.

For Wanda, that consistency has brought both peace of mind and confidence. What started as a care program has grown into a trusted relationship.

Through Chronic Care Management, patients like Wanda Goss are supported not just during appointments, but in their everyday lives, reminding them that they’re not alone on their health journey.
