Caring Beyond the Clinic: Chronic Care Team Spotlight

Suzanne Davis, LPN, has been a nurse for 21 years, and as part of the Chronic Care Management (CCM) team at Ozarks Healthcare at its Winona clinic, she plays a vital role in supporting patients with ongoing health needs.
“My role as a CCM nurse is to meet with patients to see if they qualify for our services and to explain what CCM is all about,” Suzanne said. “I spend my days making monthly calls, checking in on patients, and helping those in the clinic get enrolled. We stay on top of medications, assist with referrals, and sometimes even help with Patient Assistance Programs for different prescriptions.”
Patients say that level of support makes a meaningful difference.
One shared, “It’s good to have Suzanne here at the clinic as an extra service. She answers questions promptly, and if she doesn’t know, she finds the answer for us. She also gets my refills done quickly.”
Suzanne’s passion for nursing stems from personal experiences caring for sick family members.
“Being able to take care of and help out my community brings me a sense of completion,” she said. “I’ve known most of our CCM patients all my life, having moved to Winona in 1980.”
For Suzanne, the best part of her job is the personal connections she builds.
“They become like family,” she said. “One patient even sang ‘Happy Birthday’ to me over the phone this year - it was so special! When I see patients in the clinic, they just walk up and give me a hug. It’s the little things that make it meaningful.”
Through dedication, compassion, and a personal touch, Suzanne and the CCM team help patients stay healthy, supported, and live life to full potential.
