Caring Beyond the Clinic: Chronic Care Team Spotlight

Tamera Agee has been a LPN for 34 years, and as part of the Chronic Care Management (CCM) team at Ozarks Healthcare West Plains Family Medicine, she serves as a dedicated liaison between patients and the many physicians, clinics, and services they may need.
“My role is to connect patients with the care and resources they need,” Tamera explained. “We can connect patients with additional services, including home health and therapy, and even handle tasks like arranging follow-up appointments - sometimes literally ‘sitting on hold’ for them - so they have someone actively working on their behalf.”
Tamera’s inspiration for nursing comes from her lifelong desire to help others.
“I enjoy helping others in whatever capacity that may be,” she said. “This job allows me to form relationships with my patients and I can take the time to listen to them, and that is my inspiration.”
For many patients, Tamera’s support goes beyond typical care. “It always surprises patients that I can make calls and be on hold for them so they can focus on what matters most to them,” she said.
One patient shared just how meaningful that support can be:
“Tamera has been a God-send. I was used to doing everything on my own. A stroke or two will slow you down … she’s connected lots of dots for me, she helped straighten my medication out.”
One of the most meaningful parts of Tamera’s work is the relationships she builds.
“Many of my patients are Veterans, like me. I love hearing about their time in the military, where they served, and the experiences they’ve had. For just a short monthly call, I can brighten their day, and that means something to them. I am more than just a name on a business card, I am a friend.”
Through her compassion, dedication, and personal touch, Tamera helps patients stay healthier, feel supported, and know they are never navigating their care alone.
